Antibiotics are among the most important medical discoveries in history, transforming once-deadly infections into minor inconveniences. But the growing threat of antibiotic resistance could return us to an era where simple infections become dangerous again. Understanding how antibiotics work and how resistance develops is critical for preserving their effectiveness.
How Antibiotics Kill Bacteria
Different classes of antibiotics target different bacterial functions. Cell wall synthesis inhibitors (penicillins, cephalosporins): prevent bacteria from building their protective cell wall, causing them to burst. Protein synthesis inhibitors (tetracyclines, macrolides): block the bacterial ribosome from making essential proteins. DNA replication inhibitors (fluoroquinolones): prevent bacteria from copying their DNA. Metabolic pathway inhibitors (sulfonamides): block essential chemical reactions bacteria need to survive.
Why Antibiotics Do Not Work on Viruses
Viruses are fundamentally different from bacteria. They lack cell walls, ribosomes, and independent metabolic pathways. They hijack your own cells to replicate. Since antibiotics target bacterial-specific structures, they have no effect on viruses. This is why antibiotics do not treat the common cold, flu, or most sore throats.
How Resistance Develops
Bacteria reproduce rapidly (some divide every 20 minutes). Random mutations occasionally produce bacteria that can survive antibiotic exposure. When antibiotics kill susceptible bacteria, resistant ones survive and multiply to fill the vacated space. Bacteria can also share resistance genes through horizontal gene transfer, spreading resistance to other species.
What Drives Resistance
Overuse: prescribing antibiotics for viral infections or minor conditions. Incomplete courses: stopping antibiotics early allows partially resistant bacteria to survive. Agricultural use: antibiotics in livestock account for a significant portion of total antibiotic use. Poor infection control: resistant bacteria spread in hospitals and communities.
The Current Threat
The WHO lists antibiotic resistance as one of the top global health threats. MRSA, drug-resistant tuberculosis, and carbapenem-resistant bacteria are already causing deaths worldwide. Without new antibiotics or strategies, routine surgeries, cancer chemotherapy, and organ transplants become significantly more dangerous.

Only take antibiotics when prescribed. Complete the full course as directed. Never share antibiotics or use leftover prescriptions. Practice good hygiene to prevent infections. Support vaccination programs that reduce the need for antibiotics.
